Vsync increases the loading time to an insane degree. You need to disable vsync and use dxtory to cap the frame rate at about 50 fps (which you should be doing anyway, regardless of the loading issue). Actually, I think enabling vsync and using dxtory to cap lower than the refresh rate works too....How do I fix GTA 4 loading forever?

- Press ctrl+shift+esc.
- Click expand.
- go to startup in the top left corner.
- click on GTA 4 and click disable.
